How did the patient hear about Cortlandt: Previous patient

Details of Experience:

Clara was admitted to Phelps Hospital for a right hip fracture due to the numerous falls she had experienced. Clara had a decrease in her strength, making her unable to stand or ambulate safely. She became anxious and frightened about her risk of falling. She has been with us before, and throughout her stay, we have consistently met her expectations. It was an easy choice for this patient to return to Cortlandt.

Upon arrival, Clara was greeted by our receptionist, who welcomed her warmly. She then met the concierge and the nurse, who helped her settle into her room. Nayeli ensured Clara had dinner and prepared her for bed, so the patient could rest up for the beginning of her rehabilitation journey at Cortlandt. She was ready to regain her strength, to get back on her feet, and feel like she did before being hospitalized.

Clara was evaluated the following day by our rehab team to set up her short-term goals. She was independent with supervision. She was very outgoing and always in high spirits. She gave 100% in rehab to make her stay as brief as possible. She was able to regain her strength. After all her hard work, she could ambulate 150 feet on a level surface using a two-wheeled walker under supervision. She was able to increase her independence gradually. Clara had such a loving and supportive family who came in every day to see her. Whether it was a 5-star day or a challenging one, she did not let it stop her from meeting her goals.

Clara soon meets all her goals and has completed her rehab journey. When it came to discharge, it was always so sad to see her go. But watching her work so hard to get back on her feet and regain her strength always made us so proud of her. Clara will always be part of the Cortlandt family, and we wish her always the best.
