Details of Experience:
Ms. D arrived at the Quality Center for Rehabilitation and Healing on 07/25/2024. Ms. D had worsened neck pain and kyphosis and was admitted to Ascension Saint Thomas Midtown in Nashville, TN, on 7/17/2024 for surgery that consisted of a C2-T2 posterior spinal fusion. Ms. D was living independently prior to her surgery and coming to our community.

Upon her arrival in our community, Ms. D was greeted by several members of our professional team. Within her first 48 hours here, she met with our clinicians, therapists, physicians, administrative staff, and many other team members, who welcomed her to our community and assured her they were all here to support her recovery. She was also greeted by one of our Concierge, Cecelia Hall, who gifted her with a branded CareRite welcome bag, explained her role as liaison, answered any questions, and spoke with her at length about her life prior to her surgery and what her goals were here at Quality Center for Rehabilitation and Healing. She was expecting to return home with home health.

After her admission to our community, Ms. D was initially evaluated by Therapy on 07/26/2024 to help improve her function. Physical therapy noted weakness in her right hand and difficulty grasping objects. She was unable to ambulate and required maximum assistance with sitting, standing, and transfers. She then met with Occupational Therapy, and it was noted that Ms. D needed maximum assistance for eating, moderate assistance for upper body dressing, and was totally dependent on lower body dressing and toilet hygiene.

Unfortunately, at that time, Ms. D was not making the progress that therapy had hoped for and insurance denied her for lack of progress; she was discharged from therapy on 08/30/2024. Ms. D was on therapy’s caseload throughout the year, and they noted some improvement in her functions in November 2024. Therapy then picked her up once again, and her progress was exponentially greater compared to previous attempts. As of February 2025, Ms. D’s functional status was as follows: her transfers progressed to moderate assistance, she was ambulating with a front wheeled walker 75 feet. She was able to eat with set up assistance, upper body dressing with moderate assistance, and lower body dressing with maximum assistance.

Currently Ms. D is ambulating 150 feet with a front wheeled walker and is supervision assistance. She is feeding herself independently, upper body dressing with set up assistance, lower body dressing requires supervision, and she is performing toilet transfers and hygiene with moderate assistance.

Ms. D has been a part of our community for around 520 days. Although she will not be discharging home as expected and is going to remain in our community long term, Ms. D has been such an inspiration with her determination, hard work, and heart. She continues to challenge herself daily and wants to improve at all levels. She enjoys working out in the gym, her time with therapy, going to activities, and our company. She said she enjoys being with us and that we provide the encouragement she needs to continue her journey to recovery. It is such a joy to have you in our community, Ms. D, and we are so proud to see your continued growth and progress here at Quality Center!
