Details of Experience:

Mr. P came to our community in early October 2025. The patient was admitted from Tri Centennial Medical Center with a diagnosis of cellulitis. Upon admission, he met our nursing and therapy staff and his social worker. They all welcomed him with open arms.

Previous to admission, this patient was living independently. As such, he found it challenging to accept help and assistance from staff. Once he did, he did not want to leave. He began bonding with our staff and the other residents.

As Mr. Polk’s time with us continued, he made significant progress toward his goals. He enjoyed the food, and as a positive outcome, he gained weight. He increased his standing duration from 10 minutes to 30 minutes. He also regained his independence in activities of daily living (ADLs). Beyond the physical changes, his perspective changed as well. “Instead of looking at what I can’t do, I look at what I can do,” said Mr. P.

Jackie P was a source of light here at Nashville Center. He was very involved in activities, enjoying BINGO, live music, and simply talking to the friends he made here. Everyone was fond of his pleasant energy and smile. When reminiscing on the time he had here, he stated, “It brings tears to my eyes to see people who care so much.” Our staff were similarly teary-eyed when he was discharged.

We would like to thank Mr. P for allowing us to share his story. We are honored to be a part of his healing journey. His constant encouragement of others was a gift to both patients and staff alike. He became friends with all who worked with him. We wish him the best of luck as he continues on his healing journey.
