Raymond F was admitted to Hudson Valley Hospital following a fall at home. During his hospitalization, a need for rehabilitation was identified, and he was referred for continued care. Raymond chose to continue his recovery at Cortlandt Healthcare.
Raymond was welcomed to Cortlandt Healthcare by his Concierge to ensure he had the most comfortable stay and to support his needs in any way possible. During his stay, his condition required surgical intervention, resulting in a below-the-knee amputation. Raymond required assistance with mobility and activities of daily living. Key clinical considerations included decreased strength, mobility, and endurance, and a much-needed decrease in pain with functional activities. Despite these challenges, Raymond demonstrated excellent rehabilitation potential, evidenced by his ability to follow multi-step directions, strong family involvement and support, and positive engagement with most staff.
An interdisciplinary approach was implemented to support Raymond’s recovery. Physical therapy focused on improving strength, mobility, and endurance, and on reducing pain during functional activities; as well as occupational therapy focusing on regaining independence with ADLs (activities of daily living), including dressing, bathing, and establishing a new daily routine following prosthetic use.
Throughout his stay, Concierge Nayeli provided consistent support, ensuring Raymond felt encouraged and well cared for during his recovery. In addition, Dennis Oehler, a former Paralympic gold medalist from the CareRite Comprehensive Amputee Rehabilitation Program (CARP), visited weekly, offering companionship, encouragement, friendship, and moral support during a challenging time. This involvement played a key role in maintaining Raymond’s motivation and emotional well-being.
Raymond remained committed to his rehabilitation goals and actively participated in therapy. He worked very hard to increase his leg strength, improve mobility with assistive devices, and train with prosthetics. Regaining independence in dressing and personal care. Raymond’s primary goal was to give his full effort in therapy since day one to return home to his two beautiful girls, who provided him with strong emotional support.
Through determination, consistency, and support. Raymond made meaningful progress toward independence and recovery. Raymond F’s journey is a powerful example of resilience, determination, and the impact of a strong support system. His courage, work ethic, and positive attitude inspired both staff and fellow residents. His case serves as a reminder that even in the face of significant challenges, meaningful recovery is possible with dedication and support. Raymond remains an unforgettable resident whose strength and perseverance left a lasting impression on everyone involved in his care.
