Details of Experience:

On 05/07/2026 Mr. Allen was admitted to The Phoenix Rehabilitation and  Nursing Center with an admitting diagnosis of non-pressure chronic ulcer in his right upper leg. This resulted in Mr. Allen not able to bear any weight on his right leg due to severe pain and lack of strength in his lower extremities. Mr. Allen needed to focus on his strength, balance, endurance, bed mobility, bed transfer, and gait training. Mr. Allen was very  motivated to begin his therapy and gain independence back. His plan was to receive short term rehabilitation and discharge back home to his apartment.  

Upon admission, Mr. Allen was welcomed by our interdisciplinary team, including nursing,  therapy, social work, recreation, and concierge to ensure a smooth transition to the facility and make sure that all his needs were taken care of. He was evaluated by his physical  therapist who assessed that he required partial assistance with bed mobility and  maximum assistance to stand with a walker for support. He was also evaluated by his  occupational therapist who assessed that Mr. Allen needed to work on his range of motion  as well as activities of daily living (ADL).  

After one month, Mr. Allen showed improvement in both his physical and occupational  therapy, as well as an improvement in the severe pain he experienced when first admitted  from weight bearing. Mr. Allen improved from not being able to walk 175 feet with a rolling walker and supervision. He also improved, requiring only supervision of 1 person, to transfer from his bed to a wheelchair. 

Mr. Allen stayed determined and after about 2 months, he improved to needing just set up  assistance to transition from sitting to standing and he was able to walk 350 feet with set  up assistance as well. Finally, Mr. Allen is now able to walk up 21 stairs, while holding 1 rail with supervision. Through the hard work from our team at The Phoenix, Mr. Allen plans to discharge home to his apartment.
