How did this patient hear about Water’s Edge: Recommended by Mather Hospital

Details of Experience:
It was just an average day for Christine when she found herself experiencing weakness and dizziness in the comfort of her own home. This unsure feeling led to a tragic fall to the floor, alerting her that she needed serious medical attention. Christine headed to Mather Hospital where she was admitted for hyperparathyroidism, osteoarthritis, weakness, and dizziness. After spending some time in the hospital, Christine was recommended to continue her recovery journey by admitting to a Subacute Rehabilitation Center. The skillful team at a Subacute Rehabilitation Center would be able to ensure Christine returned home in her best condition. After careful consideration, Christine and her family felt Water’s Edge Rehab & Nursing Center was a good choice as it was close to her family.

Upon admission to the Water’s Edge Rehab & Nursing Center on December 6th Christine was greeted by a number of friendly interdisciplinary staff. On top of the Water’s Edge’s supportive staff, Christine also had an abundance of support from her husband, Peter, and children. Christine was eager to get started on her journey home. After being evaluated by her physical therapist, Jessica, and occupational therapist, Rebecca, Christine set goals to decrease her needed assistance, improve mobility, pain management and then return home with her husband. Christine’s team developed the best plan for her using physical and occupational therapy.

During physical therapy, Christine used progressive resistance exercises and therapeutic resistance exercises. She worked with her therapist on dynamic sitting and standing balance to improve her ability to perform functional mobility tasks. Christine also practiced bed mobility training and transfer training to decrease caregiver assistance and improve functional skills to get in and out of bed, along with getting in and out of her wheelchair.

During occupational therapy, Christine used open-chain kinetic exercises and therapeutic exercises to facilitate independence in self-care tasks. She worked with her therapist on gross motor coordination techniques, postural control, and facilitation of weight shift and dynamic stability. Christine also practiced bathing techniques, self-feeding techniques, and dressing techniques. Rebecca worked with Mrs. Krasowski on safety training during transitional movements to increase independence with tasks and analysis of movement during activities of daily life.

Christine started this journey off needing maximum assistance with all daily life activities. She has since met all her goals with the help of our team of professionals at Water’s Edge Rehab & Nursing Center at Port Jefferson. As her discharge day quickly approached, Christine could not be more excited to be going home with her husband as planned, needing only minimal supervision. Our team of therapists provided an in-service of adequate training so that Peter could provide Christine with the best assistance when necessary at home.

Christine and Peter were so eager and happy to be together again at home. Water’s Edge wishes her and her family nothing less than a happy and healthy future.
