How did this patient hear about Chatham Hills? Recommendation from rehab facility at Overlook Medical Center

Patient was admitted to Chatham Hills Subacute Care Center after receiving rehabilitation for his left leg at Overlook Medical Center. Patient stated, “The physical therapists at Overlook told me I followed directions so well I was given a promotion by being sent to Chatham Hills”. Upon his admission, the patient was using a wheelchair for safety purposes as he demonstrated difficulty standing and balancing. He often feared his left leg would give out at any moment and didn’t want to risk falling. He worked closely with his physical therapist, Steve, whom he described as having a “brother-like relationship” with. Steve’s instructions for exercise were demanding and incisive with a caring and friendly approach. Through exercises increasing the strength of his lower and upper extremities and progressive exercises to improve activity tolerance along with balance training, the patient improved immensely. Upon discharge, he went home with his wife relying on a rolling walker. He can also walk with the assistance of a cane. The CNA’s and aides would see him roaming from activity to activity and shout, “There he goes again! You go kid!” Patient stated, “I would recommend Chatham Hills Subacute Care Center to anyone who comes anywhere near me.”
