Resident was admitted to The Grove on 6/5 with admitting diagnosis of Syncope and Collapse which exhibits new onset of decrease in strength, decrease in functional mobility, decrease in transfers, reduced ability to safely ambulate and reduced balance indicating the need for PT and OT to increase independence with gait, facilitate independence with all functional mobility, and promote safety awareness. She did phenomenally with her therapists- day in and day out. It was tedious work, but with the right motivation and skilled staff, the progress was clearly evident.
When resident was discharged from facility on 8/5, resident met all short-term and long-term goals of completing car transfers with Modified independence, ambulate unlimited distance using a single point cane with modified independence, and the ability to complete all ADLs with increased safety and Independently.
