Ms. Khalilah was admitted to The Phoenix Rehabilitation & Nursing Center after her stay in New York Methodist Hospital due to altered mental status and congestive heart failure. Upon her admission, she had increased weakness, and pressure ulcer. In the beginning, she refused to go to therapy; she was grumpy all the time. The director of therapy tried convincing her many times. She finally said she’s going to give her one shot. She attended PT and OT. After her first session of PT and OT, she was amazed of our therapy how caring and professional they are. PT worked with the patient to increase dynamic standing balance in order to reduce the risk for falls and prepare for gait activities.
Ms. Khalilah discharged on 8/10/15, and she did therapeutic exercises, therapeutic activities, bed mobility, transfer training, balance training, gait training and safety training.
