Details of Experience:
Upon Admission, Resident exhibited a new onset of decrease in strength, decrease in functional mobility, decrease in transfers, reduced ability to safely ambulate, reduced balance, reduced functional activity tolerance, decreased postural alignment, reduced static and dynamic balance, decreased coordination, decreased skin integrity, increased need for assistance from others and reduced ADL participation placing William at a high risk for decreased ability to return to prior level of assistance. The resident also had a decreased circulatory function, a poor level of mobility, poor leisure task participation, decreased participation with functional tasks and decreased skin integrity. William had bouts of depression, falls, further decline in function, immobility, increased dependency upon caregivers, muscle atrophy, pneumonia, pressure sores and social isolation. Through a lot of hard work from our amazing clinical and Rehab team, Resident made great strides in getting a clean slate of health.
When Resident was discharged to the community his ADLs on his due care were Independent and the
Resident was discharged being able to ambulate with a cane while being able to walk unlimited distances.
