How did this patient hear about Sans Souci? Social worker in Hospital

Details of Experience:
Wadi was admitted to Sans Souci Rehabilitation and Nursing Center on September 8, 2015 from Mt Sinai Hospital. He had an admitting diagnosis of Spinal Stenosis s/p C3-C7 Anterior Discectomy and Fusion with Anterior Fixation. Wadi also had paired Vertebral Screws & Interbody Spaces, as Well as Spinal Fixation with Rod and Paired Lateral Mass Screws, HTN, and Myelopathy. When Wadi was evaluated by our therapy team he was fully dependent on all his ADL’s with a pain level of 9/10. He was transferred from bed to chair via Hoyer lift as he wasn’t ambulatory at all. It took lot of hard work and patience But Wadi pulled through! Despite having a small language Barrier with the staff, Wadi always had a smile on and the staff all loved him. After Weeks of Therapy, Wadi made tremendous progress. When he was discharged from Sans Souci, Wadi was Modified/Set-up for most ADL’s. His pain was a 1/10 and he was able to ambulate more than 200 feet using rolling walker. A major thank you to our Rehabilitation team for the great work they did with Wadi ensuring that he made a complete recovery!
