Patient is 81 years old and was admitted to Nyack hospital due to an acute bilateral pulmonary embolism with a diagnosis of hypoxic respiratory failure with symptoms of SOB, bardycardia generalized weakness. Patient came to Ramapo Manor on June 20, 2016. With the assistance of our occupational and physical therapy she has gotten much stronger, modified independence of activities of daily living dressing, functional transfers, improved balance and decreased risk of fall.
Patient has demonstrated increased strengh for all self care activities and now has good safty awareness. Patient has been here for 7 weeks and have found everyone to be extremely kind, caring and professional; we really enjoyed her during her stay here at Ramapo Manor as well as her. She will be discharged with a rollater to Dowling Senior living.
