Sydney was admitted to Sans Souci Rehabilitation and Nursing center on April 25th 2017 from Westchester medical Center with a diagnosis of cerebrovascular disease s/p Tracheostomy. When Sydney was assessed by our wonderful therapy team, he presented with poor activity tolerance and impaired balance, thereby resulting in poor functional mobility. Sydney required extensive assistance with bed transfers, functional transfers and ambulation. Sydney was able to ambulate 25 feet with a rolling walker with taking multiple breaks. Sydney was a pleasure on the floors as he was always conversing with other residents and employees. He has a loving wife who was by his side every step of the way, which was a big motivation for him to get better quickly! As Sydney worked diligently with the therapy and nursing teams, he began to make amazing progress. At his time of discharge back to the community, he was able to ambulate unlimited distances using a rolling walker as well as being able to ascend/descend 15 steps with modified independence! Sydney was able to also perform his ADL’s (Activities of Daily living) and IADL’s independently as well. He shared his appreciation for everything that Sans Souci Rehabilitation and Nursing offered him during his stay and he looks forward to living an independent life once again!
