Thomas Healy came to The Emerald Peek after suffering a minor heart attack with a family who was worried about how he would return home. When Thomas and his family arrived, he was greeted by Sandy Gomas, RN Care Manager and Katie Cullum, Director of Concierge Services.
Once settled in his room, he was greeted over the course of the next 3 hours by the rest of the clinical team, including Director of Rehabilitation Services, Director of Social Work and Director of Therapeutic Recreation. The team learned Thomas was shy, but focused on his rehabilitation. He expressed concern he would never be home again. There was no more important place for him to be than home.
Weeks 1 and 2 of the Patient’s Stay:
When Susan Agrella, Director of Social Work, met with Thomas, she noted his devotion to his faith and how important his faith would be to his recovery. Susan spoke with George, our Interfaith Minister, who met with Thomas twice a week during his stay
During the first 24 hours:
Thomas met Andrea Johnson, a Certified Occupational Therapy Assistant, who worked with Thomas on setting short term goals to enable him to be safer at home. Andrea and Thomas worked on safe grooming and hygiene tasks, including toileting, in the bathroom of the Activities of Daily Living Suite. Andrea also worked with Thomas on lower body dressing with no adaptive equipment. At the end of two weeks, Thomas was able to perform all these activities safely without adaptive equipment. Thomas’ family was very happy that no changes would need to be made to his home in order to accommodate adaptive equipment.
Within a week of Patient’s Discharge:
Thomas transitioned to home on January 14, 2018. Thomas told Tom, Director of Rehabilitation, “This is some program you have here. I have seen lots of people come into this gym and get stronger everyday. You made me stronger too and I am so grateful. Thank you for everything you’ve done for me.”
