How did the patient hear about Creekside Center? Hospital Referral

Details of Experience:
The featured Champion of the Month, Melody Akins, presented to Skyline Medical Center Emergency Department after suffering many falls, as a result of a stroke. Following a three-day observation period and treatment, it was determined that she receive continued skilled therapy and temporary assistance with activities of daily living. Subsequently, Ms. Akins was transferred to the Creekside Center for Rehabilitation and Healing.

Upon arrival, she was welcomed by our friendly staff. The Concierge visited on the day of admission, and frequently thereafter, to ensure comfort and satisfaction. The initial evaluation by therapists revealed the need to: increase functional mobility and strength, improve poor balance, enhance fall recovery abilities, and improve memory and problem-solving. The patient required assistance with all activities of daily living.

In between therapy sessions, Ms. Akins enjoyed various activities, such as bingo and Wheel of Fortune. She looked forward to group therapy sessions, as it is another opportunity for socialization. She was also excited to attend the Christmas and New Year’s Eve parties, and reports having a great time. When asked about her experience at Creekside Center, she always speaks highly of those who care for her. She expressed gratitude toward her nursing assistants for braiding her very long hair after her shower and making sure she got her second cup of coffee every morning. The employees of Creekside Center were described as wonderful and trustworthy.

Ms. Akins declared, “When I first came here, I literally couldn’t walk without feeling like I was going to fall.” She continued, “Now, I can walk for long distances without a walker or cane and I feel safe.” She reports being very pleased with her progress, in all areas. As she nears discharge, her goal of moving to a California beach townhome near her parents looks much more promising. She attributes her success to her “determined not to fail” mindset and the Creekside Center team who are diligently training her to live safely and independently. We commend Melody Akins for being a true champion and wish her a future of continued victory!
