Details of Experience:
Ms. Jimmi Lou S. arrived at the Gallatin Center for Rehabilitation and Healing on September 17, 2020 from Trust Point Rehab hospital in Murfreesboro, Tennessee. Ms. Schneider’s postoperative left hip replacement was due to a slip and fall at home. She woke up in the middle of the night and lost balance while she was walking from her bedside to her bathroom sink. She was informed by the ER team that she had a left hip fracture. The orthopedic surgeon performed hip replacement surgery to her left hip and she was able to begin her recovery.

The therapists at Trust Point Rehab focused on Acute Rehabilitation services, including emotional health, bed mobility, transfer, balance and gait training. Ms. Schneider required moderate to maximum assistance with most of her functional tasks. After 16 days of therapy, she transferred to Gallatin Center for Rehabilitation and Healing for subacute rehabilitation, to help her return back home with her daughter.

Ms. Schneider was greeted by the Interdisciplinary Team and the Gallatin Center, who excitedly awaited her arrival. Her first day was filled with evaluations from the team members and preparing her mind for the work ahead. The therapy team evaluated Ms. Schneider, created individualized goals with her focused on increasing strength, balance, and gait training to increase independence to return home safely. Her baseline for ambulating with a rolling walker was 50 feet with minimum assistance from her therapists. She required moderate assistance with bathing and dressing tasks at the start of her therapy journey at the Gallatin Center. After participating in balance training, upper and lower body strengthening, and gait training, Ms. Schneider demonstrated progress quickly.

By September 24, 2020, Ms. Schneider had increased her ambulation with a rolling walker from 50 feet to 150 feet. She increased independence with functional tasks from requiring moderate assistance to stand by assistance. She shared she enjoyed working with her therapists and the nursing staff had been so nice and helpful to her. “My room has been exceptionally clean. The housekeeping staff go over and beyond with cleaning,” Ms. Schneider stated. Four days later at discharge, Ms. Schneider added 50 more feet to her progress and walked her longest distance of over 200 feet. She only required set up for functional tasks which was one of her goals on admission. Ms. Schneider could see the end goal in sight: snuggling up with her two rescue dogs, a pug and Heinz 57 puppy.

The Discharge Planner arranged for Ms. Schneider to have continued therapy services in her home to help her transition back into her previous routine. Ms. Schneider showed determination, positivity, and resilience and was able to exceed her short term and long term goals in a short period of time.
