Joan S. arrived to our community on September 21st. She had been feeling dizzy at home for a few days and dehydrated before going to the Emergency Room. She lives alone and has an aide scheduled a few hours a day to assist her.
Upon her arrival to The Willows, she was greeted by the Concierge, Yehuda. She was introduced to the nursing team and was given a thorough evaluation. The nursing team was very familiar with Joan, as she has been here a few times in the past. She was evaluated by the therapy team, including speech, physical and occupational therapists. She was happy to see the familiar faces again in the therapy team.
The teams formed their goals and plans for getting Joan back to her apartment, as soon as possible. The physical therapy team focused on returning her to independence. When she arrived, she was only walking 10 feet. In a few weeks, she was walking over 150 feet, independently. The occupational team worked on her ADL’s. When Joan arrived, she was moderate assistance for most activities of daily living. Within a few short weeks, she was readying for discharge, and she was quite close to being independent. The teams supported Joan in helping to manage her diabetes. This part was crucial because her diabetes had sent her to the emergency room numerous times.
Joan was visited daily by the Concierge, Yehuda. She always had a nice word for everyone. The Concierge helped her set up a roku player so she would be able to watch all of her favorite shows. She really appreciated the ability to be connected and not have to miss anything.
Upon her discharge, Joan returned to her own apartment. She still has her aide come to assist her a few hours a day. The therapists feel that she is a safe discharge and can manage her diabetes.
She will be missed but she has been at The Willows before and knows there is only one place to consider when she has a need.
