Details of Experience:
Herman is a 61-year-old man who has experienced multiple falls. His last fall happened in the night and he was found by a neighbor the next morning and called an ambulance. The patient was transported to Good Samaritan Hospital. He came to our community on January 31st. Herman lives alone and has a concerned neighbor that checks upon him from time to time.

Upon his arrival to The Willows, Herman was greeted by the Concierge, Yehuda. He was introduced to the nursing team and was given a thorough evaluation. He was then evaluated by the therapy team. This includes speech, physical and occupational therapists.
The teams formulated their goals and plan of care for getting Herman back to his house. The physical therapy team was focused on getting back to his independent level. Their biggest hurdle was the swelling in the legs, confusion, and poor safety awareness. When he arrived he was a mid-max assist for everything.

The occupational therapy team worked on building up his upper body strength and ADL’s. When he arrived at The Willows, he needed moderate assistance going to the bathroom and was dependent for lower body dressing. When he left he was modified independent for everything, the OT team educated him on safety awareness and energy conservation.

Herman was visited daily by the Concierge, Yehuda. He always had nice words for everyone. He always had a smile on his face and was very thankful. Upon his discharge, Herman went back to his home. He is confident and feels strong enough that he won’t have any more falls.

If he does need therapy in the future, he knows there is only one place to go.
