Details of Experience:
Mr. Simmons went to the hospital after multiple falls at home. Upon arrival at the hospital, they found he had a hematoma on his lower back, contusions on his pelvis and lower back, and injury to the right hand and right shoulder. He also has a history of Parkinson’s. Upon arrival at Lebanon Center, the therapy team immediately began assessing and working on a plan to get Mr. Simmons back to performing daily functions independently.
When coming to our facility, Mr. Simmons could only walk 25 feet with max assist at his admitting evaluation. He can now walk 250 feet with the minimal assist. He also required two people to transfer. He can now transfer with the help of one. Mr. Simmons was also able to stand for less than two minutes initially and now can stand for five to ten minutes.
Improved function of Mr. Simmons’s left arm has been seen. He was dependent on all dressing and self-care needs. He now can participate in dressing himself and bathing himself with just a minimum of one person.
Mr. Simmons started out staying in his room…closed off to activities and others. Mr. Simmons now eats in a more social gathering while staying socially distant. He will be returning home to his wife and beloved puppies.
