Details of Experience:
Ms Johnson was admitted to The Phoenix Rehabilitation and Nursing Center on January 30th, 2021 from The Brooklyn Hospital Center, where she had been admitted due to synovectomy, debridement, and revision of the left knee. During the first 24 hours in the community, Ms. Johnson was welcomed by the amazing concierge team and social worker.

The patient’s first week in the community helped her open up slowly to the clinical team and other patients. She had slight difficulties adjusting her foot and did not have much strength in her knee. As she began rehabilitation, she quickly found her two favorite physical therapy staff members, Serry-Ann and Joycelyn Victor. As Ms. Johnson began her therapy, she was very unmotivated and felt like she would not be able to bend her knee again. However, with everyone being so supportive, she began to push herself towards her goals.

Ms. Johnson really enjoyed having engaging conversations with our concierge team about how she was planning to travel. She also loved receiving supplies from activities throughout her stay, which kept her motivated and busy. She really enjoyed painting and coloring, which our recreation teams were more than happy to provide. She loved fashion and music, so the concierge provided magazines. Her fantastic social worker was able to provide emotional support and kept her family updated.

Throughout the first month of Ms. Johnson’s stay, she progressed very well. She became more engaged and determined. She went from not being able to get out of bed without assistance to getting herself out and doing daily activities like walking through the hallways and walking upstairs. She continued to come down to the gym with Sherry-Ann and continue with additional therapy. 

On the last week of Ms. Johnson’s stay, she became emotional about leaving the community. She became very comfortable with the staff and considered them her family. The concierge gave her a bag with goodies for her to take with her, so she could always have a piece of The Phoenix close to her. Her social worker was able to connect her with home care services through a certified home health agency. Rehabilitation provided her with supplies that she would need at home for assistance.

On March 12, 2021, Ms. Johnson was discharged to her home with support from her mother. She will be receiving home care services to improve further the rehabilitation that started here at The Phoenix.
