Details of Experience:
Mrs. Ingram came to our facility from Vanderbilt Wilson County hospital. She was admitted there due to COVID-19 complications. Mrs.Ingram had severe shortness of breath when she came to our community following her recovery from COVID-19. 

She had increased activity tolerance to be able to complete activities with increased independence. She needed maximum assistance for toileting and ADLs on admission. Before her admission Mrs. Ingram was able to complete all daily functions on her own with no assistance. Mrs. Ingram was also unable to ambulate over five feet on admission. She also required moderate assistance for transfer upon admission. 

Mrs. Ingram was very active with activities while she stayed at Lebanon Center. She participated in bingo, as well as other activities she really enjoyed taking part in. She enjoyed social distance gatherings in the common area during the day as well. She was able to progress at an accelerated rate and was at her full potential in a matter of weeks. 

Mrs. Ingram was discharged from our facility and able to walk 150 feet with a rolling walker. She was discharged home at an independent level.

We are so glad Mrs. Ingram was able to make a quick recovery and return home independently.
