Details of Experience:
Ms. Gracy came into our facility with a left hip fracture. She had a fall while in an assisted living facility and required help from our therapy team to get back to her maximum potential. We did not doubt that Ms. Gracy would make a speedy recovery with a positive attitude. Our therapy and nursing staff quickly developed a plan to get Ms. Gracy back to her full potential.
Upon admission, occupational therapy evaluated her and determined that she was standby assistance for upper body dressing, max assists with lower body dressing, max assistance with toileting, and bathing. She could do these tasks independently with very little assistance before her fall. Physical therapy’s evaluation determined that she required max assistance with bed mobility and transfers. Her gait transfer, as well as her wheelchair propel, was nonexistent. These are things she would require to return to her assisted living, which she called home.
Ms. Gracy was a delight for therapy to work with and always tried her very best. Upon discharging our facility, she was able to dress with min assistance, toilet with mod assistance, bathing with min assistance. Her bed mobility, transfers, and gait were minimum assistance upon discharge.
Ms. Gracy was able to discharge back to her ALF memory care. She was a delight to have and stated, “I really enjoyed my stay, the staff was very accommodating.”
We truly do wish her the best in the future.
