Details of Experience:

Upon admission to our community, Ms. Williams was evaluated by our therapy and our nursing department. It was noticed by therapy that the patient had a lack of motivation to participate in daily therapy activities. If the patient hoped to go home following her discharge, this would pose a challenge.

Ms. Williams was able to perform some upper body dressing alone, she was a max assist for her lower body dressing. Her toileting, transfers, and ambulation were all also max assist. Her bilateral upper extremity was a 4 out of 5. The patient was very anxious about coming to rehab. However, Ms. Williams did a 180 and began participating in therapy in order to meet her goals and be able to discharge home. She made amazing strides to her goals over the last part of the twenty days at Lebanon Center.

Upon leaving our community, she was independent with upper body dressing; she was stand-by assist with lower body assistance. When it came to toileting and ambulation, Ms. Williams was at a supervision level. She was able to do all the essential hygiene functions that she was unable to accomplish alone when she was admitted. All this with the help of our excellent therapy team and the patient’s determination. Upon discharge, her bilateral upper extremity was also a 4+ out of 5, which is amazing.

We truly enjoyed having her in our community, whether it be in our rehab gym, residents lobby, where she made many friends, or our salon having a spa day.

We genuinely wish Ms. Williams the best since discharging home with her son.
