Details of Experience:

Mr. Howard was admitted to The Phoenix Rehabilitation and Nursing Center on May 6, 2022, from Wyckoff Heights Medical Center, where he was initially admitted for a left knee replacement. During his first 24 hrs in the community, Mr. Howard was welcomed by each of our amazing departments, such as Nursing, Assigned Social Worker, Concierge, and Recreation Therapist, with emotional support offered during his stay.

During his first week in the community, Mr. Howard began Physical and Occupational Therapy. His goals were to be able to have pain-free ambulation and ambulate on leveled surfaces/household distances of 200 ft with minimal to no assistance and to continue his daily activities once discharged. At first, Mr. Howard was very unmotivated and depressed because he felt no progress was made. However, our Concierge and Recreation team were able to visit him daily to provide emotional support and reassure/motivate him for his therapy sessions. As time progressed, he looked forward to participating in the therapy sessions he received daily throughout his stay. During the hours he did not receive therapy, he would participate in our recreational activities/lounging in our outdoor area under the pergola. While doing so, Mr. Howard was ambulating with the assistance of a cane remaining under pain management.

Throughout Mr. Howard’s last week at the community, he excelled with his therapy sessions and grew a great bond with the Therapy Department and shared that it helped him to be able to have pain-free ambulation now and ambulate over 350 ft independently or with the help of a rollator if he chooses, which was his initial goal to get back to doing things he loved.

After discharge, The Concierge Team reached out, and Mr. Howard was very pleased with the services he received and shared that he is adjusting to his daily activities well.
