Ms. Jackson was admitted to The Phoenix Rehabilitation and Nursing Center on August 23rd, 2023, from Methodist, where she was initially admitted after complaining of shortness of breath, causing her to be unable to walk long distances.

During her first 24 hours at The Phoenix, Ms. Jackson was anxious but became very open to being in a new setting. During this time, she was greeted by our multidisciplinary team and quickly began to form bonds with multiple team members. At this time, she felt very welcomed and motivated to complete her therapy journey.

During the first week at The Phoenix, Ms. Jackson took things with ease, starting her therapy journey. Upon admission, the therapy team evaluated her. Therapy began with small steps by encouraging her to do simple exercises in the comfort of her room, doing range of motion activities and breathing exercises. During Ms. Jackson’s stay, she was very engaged with the staff on her floor, and with the help of our recreation department, she looked forward on a daily basis to upcoming activities or in-house entertainment. Also, she shared that what helped motivate her during her rehabilitation goals was communicating with other patients with similar conditions and getting the chance to dine with others in the patient dining room. She also created solid and long-lasting bonds with both patients and team members.

Her goal was to be able to ambulate more than five feet without being short of breath quickly. Before starting therapy, Ms. Jackson could only ambulate approximately five feet with the help of her rollator walker. She needed maximum assistance with an unstable balance and a risk of falling. However, during her daily therapy sessions and breathing exercises, and with staff’s help, she can now ambulate 150 feet with supervision and go up and down 12 steps with/without a rail under limited to no supervision. During her stay, Ms. Jackson continued her therapy five times a week. She spent the rest of her time participating in activities and falling in love with our UrbanZen program, which offered fantastic aromatherapy to help ease the mind.

During the last week at The Phoenix, Ms. Jackson grew bittersweet about leaving behind all the friends she made during her time and all the bonds she had formed, and luckily, she was able to exchange numbers to stay connected. Our social work team began preparing discharge and scheduling any needed home care, along with therapy and ordering the necessary home equipment for her care to ensure support was available. Ms. Jackson wrote 200 heartfelt rays of sunshine cards to each person she encountered and made her stay with us comfortable.
